偏頭痛治療

Botox® 保妥適® 慢性偏頭痛(頭痛)治療於 2006 年由黃家聲醫生(Dr Garsing Wong)在奧克蘭美容醫科診所有限公司(Auckland Cosmetic Medicine Clinic Ltd)莎菲爾美容醫科診所 (Sapphire Appearance Medicine Clinic)推出。
黃醫生是 Botox®保妥適®(A 型肉毒桿菌素,又稱 Onabotulinumtoxin A)創新應用的先驅之一,他曾於國際會議中介紹多項美容與醫療上的創新應用技術。

自 2006 年起,黃醫生在為皺紋與非手術面部塑形進行 Botox® 保妥適®治療時,觀察到其對慢性偏頭痛患者的有益效果。此療法可協助患有慢性與重度偏頭痛的人減少頭痛次數與藥物需求。

什麼是慢性偏頭痛?

慢性偏頭痛是一種極具破壞性的疾病,據估計在紐西蘭影響多達 65,000 人(根據國際專家、美國西雅圖神經科醫生 Dr Sheena Aurora 於澳紐偏頭痛研討會中的報告,其盛行率為 1.3%)。

慢性偏頭痛的定義為:
在過去 30 天中有超過 15 天出現嚴重頭痛,每次持續超過 4 小時,且在過去 3 個月內有超過 8 次發作,或曾被診斷為偏頭痛,這些頭痛可能為搏動性、中度至重度、單側發作,並因日常活動加劇,伴隨噁心/嘔吐或畏光畏聲,且無其他已知疾病可解釋(此定義來自 國際頭痛分類第 2 版(ICHD-2))。

 

黃家聲醫生認為偏頭痛是一種感官過敏障礙,而不僅僅是嚴重的頭痛。
他建議患者思考「第一次頭痛發生時,你的生活中發生了什麼事?」這樣的問題。

若 Botox® 保妥適®被判斷為對您最合適的療法,將會為您量身打造一套治療計畫。

患者應保持實際期望值。根據全球神經科醫師共識,成功的定義為:
偏頭痛的頻率或嚴重程度減少至少 50%。

臨床研究證實,接受一年以上的 Botox® 保妥適®重複注射療程,有超過 70% 的患者達到這一標準。

建議所有重度慢性偏頭痛患者配合積極的物理治療,以進一步提升療效。

 

我的皺紋會因此消失嗎?

這取決於您接受 Botox® 保妥適®的部位。
部分患者在接受 Botox® 保妥適®改善皺眉紋或額頭紋時,初期可能出現輕微頭痛,通常服用 Panadol® 即可緩解。
幾小時到幾天內,一些患者會注意到偏頭痛也獲得了改善。

 

這種現象亦常見於患有**磨牙症(夜間磨牙)**的患者。
將 Botox® 保妥適®注射於咬肌區域常可有效緩解壓力型頭痛。

 

療效有保證嗎?

無法保證。建議您保持開放心態。在考慮 Botox®保妥適® 作為慢性偏頭痛療法前,請確保您已嘗試過所有傳統治療方式。PREEMPT 研究(第三期慢性偏頭痛預防性療法研究)是一項隨機臨床試驗,結果顯示使用 Botox®保妥適® 後,超過 70% 的患者頭痛次數或嚴重程度下降至少 50%。

在 Auckland Cosmetic Medicine Clinic Ltd - Sapphire 慢性偏頭痛診所,黃醫生結合有效物理治療的療程,對本診所患者的改善率超過 90%(未公開資料),遠高於研究文獻報告。

 

符合資格怎麼辦?

治療費用是多少?

若您符合慢性偏頭痛患者的標準,並曾接受預防性治療藥物、神經科醫生評估或曾做過腦部影像檢查,或由您的家庭醫師轉介,則會在諮詢當天提供 Botox® 保妥適®治療方案報價。

慢性偏頭痛的療程一般為期 1 年,期滿後會重新評估您的病情。
費用不包含黃醫生的諮詢費與物理治療費,且目前此項治療未獲醫療保險給付。

 

Botox® 保妥適®慢性偏頭痛療法能與其他偏頭痛藥物搭配使用嗎?

簡單回答:可以。
不過仍須由黃醫生進行詳細評估,以確認是否適合您的情況。此療程可與物理治療結合,為慢性偏頭痛患者帶來臨床上顯著的改善,降低頭痛的頻率與強度,並減少所需藥物的劑量。

You may feel the pain from your abdomen to your backgallbladder problems. Wegovy® may cause gallbladder problems, including gallstones. Some gallstones may need surgery. Call your healthcare provider if you have symptoms, such as pain in your upper stomach (abdomen), fever, yellowing of the skin or eyes (jaundice), or clay-colored stoolsincreased risk of low blood sugar (hypoglycemia), especially those who also take medicines for diabetes such as insulin or sulfonylureas. This can be a serious side effect. Talk to your healthcare provider about how to recognize and treat low blood sugar and check your blood sugar before you start and while you take Wegovy®. Signs and symptoms of low blood sugar may include dizziness or light-headedness, blurred vision, anxiety, irritability or mood changes, sweating, slurred speech, hunger, confusion or drowsiness, shakiness, weakness, headache, fast heartbeat, or feeling jitterykidney problems (kidney failure). In people who have kidney problems, diarrhea, nausea, and vomiting may cause a loss of fluids (dehydration), which may cause kidney problems to get worse. It is important for you to drink fluids to help reduce your chance of dehydrationsevere stomach problems. Stomach problems, sometimes severe, have been reported in people who use Wegovy®. Tell your healthcare provider if you have stomach problems that are severe or will not go awayserious allergic reactions. Stop using Wegovy® and get medical help right away, if you have any symptoms of a serious allergic reaction, including swelling of your face, lips, tongue, or throat; problems breathing or swallowing; severe rash or itching; fainting or feeling dizzy; or very rapid heartbeatchange in vision in people with type 2 diabetes. Tell your healthcare provider if you have changes in vision during treatment with Wegovy®increased heart rate. Wegovy® can increase your heart rate while you are at rest. Tell your healthcare provider if you feel your heart racing or pounding in your chest and it lasts for several minutesdepression or thoughts of suicide. You should pay attention to any mental changes, especially sudden changes in your mood, behaviors, thoughts, or feelings. Call your healthcare provider right away if you have any mental changes that are new, worse, or worry youfood or liquid getting into the lungs during surgery or other procedures that use anesthesia or deep sleepiness (deep sedation). Wegovy® may increase the chance of food getting into your lungs during surgery or other procedures. Tell all your healthcare providers that you are taking Wegovy® before you are scheduled to have surgery or other procedures The most common side effects of Wegovy® may include: nausea, diarrhea, vomiting, constipation, stomach (abdomen) pain, headache, tiredness (fatigue), upset stomach, dizziness, feeling bloated, belching, low blood sugar in people with type 2 diabetes, gas, stomach flu, heartburn, and runny nose or sore throat.
