Wegovy®
(諾和盈®) 減重

我們歡迎 Novo Nordisk 推出的減重良方 Wegovy® (諾和盈®) 上市。Wegovy® (諾和盈®) 適用於成人,作為低熱量飲食與增加體力活動的輔助療法,用於慢性體重管理(包括減重與體重維持)。適用對象為起始身體體重指數(BMI)為:

  • ≥30 kg/m²(肥胖),或
     

  • ≥27 kg/m² 至 <30 kg/m²(過重),且合併至少一項與體重相關的病症者。
     

Wegovy® (諾和盈®) 的使用方式

Wegovy® (諾和盈®) 每週注射一次,可於一天中任何時間施打,不受是否進食影響。須皮下注射於腹部、大腿或上臂部位。

Wegovy® (諾和盈®) 的費用說明

Wegovy®  (諾和盈®) 不在政府補助範圍內。您需要預約醫師諮詢預約, 諮詢費用為$150/次。醫師將評估您是否適合使用此藥物及使用方法。

Wegovy®  (諾和盈®)有副作用嗎?

透過逐步增加劑量,可減少許多常見副作用的發生。
非常常見的副作用包括:頭痛、噁心、嘔吐、腹瀉、便祕、腹痛與疲倦。
其他常見副作用包括:低血糖、頭暈、胃炎、胃酸逆流、脹氣、口乾、腹脹、膽結石、掉髮與注射部位反應。
不常見副作用包含:心跳加快、胰臟炎、胃排空延遲、血中澱粉酶或脂肪酶升高。
極為罕見的副作用為藥物過敏反應(過敏性休克)。

Wegovy®  (諾和盈®)的科學原理為何?

Wegovy® (諾和盈®)(司美格魯肽)是一種 GLP-1 類似物 ( glucagon-like peptide-1, 胰高血糖素樣肽-1),與人體內源性 GLP-1 具有 94% 的序列同源性。
其為 GLP-1 受體促效劑,可選擇性地結合並活化 GLP-1 受體,即人體 GLP-1 的作用靶點。

GLP-1 為一種生理調節因子,具有多重作用,可調節血糖與食慾。這些作用主要透過胰臟與腦部的 GLP-1 受體實現。

臨床研究顯示,司美格魯肽(Wegovy®, 諾和盈®)可降低能量攝取、增強飽足感與飲食控制能力、降低飢餓感與食慾的頻率與強度。

DEXA(雙能量 X 光吸收儀)評估結果顯示,使用 Wegovy® 治療後,體脂減少程度大於瘦身時的體脂減少,與安慰劑相比,體脂成分獲得顯著改善,且伴隨內臟脂肪明顯減少。

Wegovy® (諾和盈®)的療效如何?

在 68 週的臨床試驗中,使用 Wegovy® (諾和盈®)的體重減輕效果從早期即開始出現,並持續至治療結束。
第 68 週治療結束時,Wegovy® (諾和盈®) 相較於安慰劑的體重減輕效果更加顯著且具有臨床意義。

更多使用 Wegovy® 的患者達成了 ≥5%、≥10%、≥15% 與 ≥20% 的體重減輕目標。
在試驗初期有糖尿病前期的患者中,使用 Wegovy® (諾和盈®) 的 84.1% 恢復為正常血糖,而使用安慰劑者僅為 47.8%。

研究延伸至治療後的 52 週觀察期,共 327 名完成主試驗並維持使用 Wegovy® (諾和盈®)  或安慰劑的患者參與。延伸試驗期間未提供生活型態干預,僅安排四次門診追蹤。
在第 68 週至第 120 週的非治療期間,兩組的平均體重均略有回升,但 Wegovy® (諾和盈®) 組患者的體重仍比起始值低 5.6%,而安慰劑組僅為 0.1%。

Wegovy® (諾和盈®) 是否比 Saxenda® (善纖達®) 更好?答案是:是的。

在一項為期 68 週、隨機、開放標籤、安慰劑對照的研究中,338 名患有肥胖(BMI ≥30 kg/m²)或過重(BMI ≥27 至 <30 kg/m²)且合併至少一項與體重相關疾病的患者,被隨機分配至:

  • 每週注射一次司美格魯肽 Wegovy® (諾和盈®)
     

  • 每日注射一次利拉魯肽 Saxenda® (善纖達®) 3 mg
     

  • 安慰劑組
     

研究結果顯示,使用 Wegovy® (諾和盈®) 每週一次治療 68 週後,在體重減輕方面具有明優於 Saxenda® (善纖達®) 的效果。

在第 68 週,Wegovy® (諾和盈®)組平均體重減少幅度最大,Saxenda® (善纖達®)次之,安慰劑組最小。

其中 37.4% 接受 Wegovy® (諾和盈®) 治療的患者減重 ≥20%,而使用 Saxenda® 的僅為 7%。

Wegovy® (諾和盈®) 的效果會受到腎臟或肝臟功能影響嗎?

若僅為中度功能受損,則不影響使用。

Wegovy® (諾和盈®)與癌症是否有關?

在啮齒動物身上觀察到的甲狀腺 C 細胞良性腫瘤,屬於 GLP-1 類藥物的類別效應。

What are the possible side effects of Wegovy®?Wegovy® may cause serious side effects, including:inflammation of your pancreas (pancreatitis). Stop using Wegovy® and call your healthcare provider right away if you have severe pain in your stomach area (abdomen) that will not go away, with or without vomiting. You may feel the pain from your abdomen to your backgallbladder problems. Wegovy® may cause gallbladder problems, including gallstones. Some gallstones may need surgery. Call your healthcare provider if you have symptoms, such as pain in your upper stomach (abdomen), fever, yellowing of the skin or eyes (jaundice), or clay-colored stoolsincreased risk of low blood sugar (hypoglycemia), especially those who also take medicines for diabetes such as insulin or sulfonylureas. This can be a serious side effect. Talk to your healthcare provider about how to recognize and treat low blood sugar and check your blood sugar before you start and while you take Wegovy®. Signs and symptoms of low blood sugar may include dizziness or light-headedness, blurred vision, anxiety, irritability or mood changes, sweating, slurred speech, hunger, confusion or drowsiness, shakiness, weakness, headache, fast heartbeat, or feeling jitterykidney problems (kidney failure). In people who have kidney problems, diarrhea, nausea, and vomiting may cause a loss of fluids (dehydration), which may cause kidney problems to get worse. It is important for you to drink fluids to help reduce your chance of dehydrationsevere stomach problems. Stomach problems, sometimes severe, have been reported in people who use Wegovy®. Tell your healthcare provider if you have stomach problems that are severe or will not go awayserious allergic reactions. Stop using Wegovy® and get medical help right away, if you have any symptoms of a serious allergic reaction, including swelling of your face, lips, tongue, or throat; problems breathing or swallowing; severe rash or itching; fainting or feeling dizzy; or very rapid heartbeatchange in vision in people with type 2 diabetes. Tell your healthcare provider if you have changes in vision during treatment with Wegovy®increased heart rate. Wegovy® can increase your heart rate while you are at rest. Tell your healthcare provider if you feel your heart racing or pounding in your chest and it lasts for several minutesdepression or thoughts of suicide. You should pay attention to any mental changes, especially sudden changes in your mood, behaviors, thoughts, or feelings. Call your healthcare provider right away if you have any mental changes that are new, worse, or worry youfood or liquid getting into the lungs during surgery or other procedures that use anesthesia or deep sleepiness (deep sedation). Wegovy® may increase the chance of food getting into your lungs during surgery or other procedures. Tell all your healthcare providers that you are taking Wegovy® before you are scheduled to have surgery or other procedures The most common side effects of Wegovy® may include: nausea, diarrhea, vomiting, constipation, stomach (abdomen) pain, headache, tiredness (fatigue), upset stomach, dizziness, feeling bloated, belching, low blood sugar in people with type 2 diabetes, gas, stomach flu, heartburn, and runny nose or sore throat.
